FWIW
handbrake ver. 9.3 came out in Jan 09, I think.
It will also rip titles and chapters out of DVD image files, such as ISO. Ver. 9.2 only ripped DVD discs themselves.
The only options I ever tried were
1. NORMAL for M4V
2. CLASSIC for MP4
output worked ok in
Real Player
Quicktime
VLC
Windows Media Player
but MP4 codec in Windows Media Player (MS) only played audio, could see no video

P.S. 9.3 may also include built-in decryption, but I have not tried it.
